Remember to select a minimum of three different part numbers of cases or kits per order to get a discount of 5%, otherwise it is the tier price per contract.

If you would like to change or opt-out of the program, provide 30 days notice via email to customer service at [email protected] .
Alternatively, you can use a new form to make any corrections to your program.
